I have a voting application built with PHP a few years ago. It worked perfectly until I migrated from one server to another. Since then, when I go to add a question to the session to put it to a vote, some answer options appear that I have not assigned or that perhaps I assigned but in questions from previous sessions; It is not just taking the response options that I set, but those additional ones appear.
