Two PDF Forms from Word Documents

I have two word documents that need to be created into two separate PDF documents. Work Order Form and a Rental Application. The work Order form is extremely simple and only has 20 lines. The rental Application has quite a bit more as its similar to a loan application for rent or mortgage. Both forms must have all fields "writable"...in other words both forms will be integrated into a WordPress website and when a client clicks on the form it will open up in a typical Adobe window and the fields will be "Fillable" , then of course the client will fill out the form..print it if they need a copy, and/or email it on the fly within the Adobe window...these are all typical for a PDF window...but the fields MUST have the Text Field capability. Last but not least....if anyone has an idea on how to configure these pdf's to have the ability to be faxed as well will be a bonus. I've read quite a few articles on the subject and it sounds as though a WordPress and/or PDF with Adobe may have the capability but it sounded pretty complicated. If you have any ideas I'm open to hear them. You must have examples PDF form examples that have fillable text fields...email me an example and email me one that is live on a website. You must have both examples.
