Email Marketing - design, send and manage

We have a small database of email addresses which we are currently compiling/cleaning. Anticipate approx 1500-2500 names, we will maintain and grow this list. We require a company to put together a nice email (ie html) campaign together for us and send to this list. You can use a template but must be professional looking, we can supply graphics, logo, copy, etc. We require you to send to everyone on the list. We may ask that each email is personalized with the customers name. We are a professional company and will offer the user the ability to opt out of the list. The email should appear to come from us and replies come directly to us. We would like good statistics on the campaign. We also want a list of bad email addresses so that we may clean up our list. Please include any details and recommendations for managing this small database. (We like the features of constant contact but do not want to pay the monthly costs.) Please include samples of 2-3 email campaigns that you have completed. Websites and other graphic work are not required, don't send a long list. I just want 2-3 samples of previous email campaigns.
