Outlook plugin to send email content to a web app

We have a Web Application that needs an Outlook plugin. The plugin needs to accomplish this functionality: provide a button that, when clicked, asks to enter the "case number" for the email and sends the email content, the attachments, the login token and the manually entered "case number" via POST to a specific address. The plugin needs to work under Outlook 2003 and up.
