I am looking for a skilled Wordpress developer to create an interactive survey form for my website. The ideal candidate will have experience in creating custom forms. Initial draft of code is provided. The form will include a combination of both multiple choice and open-ended questions. The developer will need to ensure that the form is easy to use and navigate for users. We need to collect 5 primary figures to include - original Loan Amount, Start Date, Repayment Amount, Outstanding amount. (See excel file) We then apply some calculations to these numbers and present a result. The Calculations include Amount repaid to date (loan payments x number of monthly payment made to date) When the RESULTS are presented the user should be offered 3 options EMAIL RESULTS, TEST AGAIN or SUBMIT DETAILS Test AGAIN Clears form REQUEST CALLBACK - capture Name/ Phone/ Email The form is to be embedded today in a new page on my wordpress site, following the style of the current site.
