Redesign look of web forms

This is a small job on a tight budget. Basically I have two web forms that I want re-designed to give them a nice professional look. Please refer to the attached file to see details of my existing web forms. The job is for changing the look and style, color scheme and format of the forms only, i.e. the cosmetic only. There're no additional changes to the functionality of the forms (which I've already programmed in). I will supply winning bidder with the original scripts to work on.
