I would like someone to create a one page SSL website that will collect information on from a client and write the responses on to standard tax application pdf fillable form. The the back end of the site should be able to fill out the responses on the standard fillable pdf form giving clients the ability to download the forms or email the forms to themselves. It should also accept payments before the form can be generated. We also want the ability to be able to add questions to the question and determine the responses to be written on the form based on responses to the question ( Reason for applying and Questions 1 to 6)

I have recently completed several projects doing dynamic PDF generation. I have also done one before which is exactly like you want where it fills out tax documents. I can setup the back office as descried so you can make your own modifications and questions. The PDF will be stored securely online and also can be emailed as described. Also, check my portfolio, I have worked with over a dozen payment processors and have script snippet examples and can easily setup that portion for you as well. We have over 6 years experience creating dynamic PDF documents (check our portfolio and you will see where we did such projects long ago. Also, on our website we offer dynamic PDF creation as a service). We will write the code in php using the FDPF library which is free (open source).
