Automatically create new page from php template

I have a Form with fields on it such as, name, last name, address, zipcode, city, etc. This Form is connected with a payment gateway and is working correctly. This Form also consists a price, products description and an URL field. And the Form gets this data from the database. The job is to create a script that allows me to create extra Forms, just like the one I already have, but with different prices, product descriptions and URL’s. You can say that my existing Form can be used as a template that should be duplicated. So that a user can make multiple payment Forms with different prices, product descriptions and URL’s. I want it to work a bit like Wordpress is creating new blog posts: in Wordpress you are able to fill in a title for your blog post + some text in the editor field and after saving it you have a new created page. I want my Forms to work a bit in the same way. In other words I want a user to be able to give up a Name for the new payment Form and subsequently fill in the new price, new product description and new URL and after saving it a totally new payment Form is created. I want the user to be able to create as many payment Form as he want. These new prices, products descriptions and URL’s should be stored in database.
