CoRegistration Incentivisation Site

Hello, I require a website to register users to a database, and offer the chance for users details to be submitted to "Co-registration" offers at the time the user submits their details. These will be through check-boxes. I also require all users to be entered into a weekly draw, so I can access this on the admin panel - and see "Names, Email addresses,Entries, etc" of those who have been entered. The more boxes ticked by the user the more entries they get into the weekly draw. I need to be able to view all the information submitted by the user in a database, preferably all in one row.
