I want to make an online survey that rates 15 products and then ranks them in a professionally looking table. Each product has 25 attributes i.e. values, 20 of these are ratings (a number between 0-5). The survey is going to consist of 24 questions;
The first two are “Are you a man or woman” and “What’s your age” (multi-choice)
The next two are multi-choice question which sort of values to exclude from the survey.
The next 20 questions will have 4 options, each representing a multiplier (A: 20x B: 15x C: 10x D: 0x). These questions are directly connected to the 20 values mentioned above (for each product).
When the answers have been checked then the pollster has to click a button which multiplies the answers with their respective values from the database. The amounts are then added to a sum for that product. This goes for every product. The results are shown in a new frame, sorted in a nice table with the right image shown for each product eg:

Last of all, the pollster should have the option to see additional details about the products he/she is interested in by selecting them (using a checkbox?) and then selecting from which database to extract the details. There are only a few details for each product so this can be shown in a very basic table on a new page.

Previous experience in development of an on-line survey website would be highly regarded. Successful bidders must be able to demonstrate ability with sample project, and/or aspect of the above requirements.
