Need someone that can write action script for existing HTML form

I have a form that is already done in Dreamweaver. Only thing is I need someone to write the action script to plug into it to make it work. I can write simple PHP scripts, but I'm having trouble with this one b/c it has upload fields so people can attach pictures/documents. I need this job done ASAP. Please only contact me if you can finish this in one day. The form is not very large. 25 fields total including 11 upload file upload fields, a few drop down menus, and all the rest are simple text fields. Should not be too difficult for someone good with forms. My budget for this project is very small.
