JavaScript API checkout for UltraCart

I need to have 2 part web order form developed using JavaScript API checkout for our UltraCart shopping cart. What I need to achieve is to for my customers to fill out a 2 part web-order form on my website. The first part would capture their name, phone and address. the next page would capture their credit card type, number, exp-date, and security code. once the credit card is validated, this would all trigger a purchase of one of our items in our UltraCart account. an example of how I want my page to work is here: [login to view URL] All of the web pages and form fields are built, we just need the form code written to communicate with our UltraCart account.
