Simple Facebook upload photos application script

Hi guys, i need a simple facebook application script using latest sdk which does this -get access token(i need extended access token as well) -get publish stream permission -uploading a photo to user album (file from my server) with description that's all i need, it should be very easy task especially for those who had experienced already in this it would be nice if u have example before bidding please don't bid if u don't understand what i need thank you
