application - IDE - Form Builder - jax

Project is simple in concept: a form builder application that is WYSIWYG. supports remote connection to mysql, mssql database, which allows tables to be used as the source of a form. supports building a form from scratch without a database needed. supports jquery for form design (or not). supports various methods for layout including tabs, collapse, as well as chained dropdown menus. supports validation methods. supports template ability (all JS saved seperated and included) cross browser / os support all WYSIWYG & drag-n-drop. support / include at least 2 different forms of captcha also support css as well as ability to edit css. support various alerts for failed validation: 1) form element should change color (selectable in ide) 2) message next to field (selectable via ide) ability to support date / time popouts should generate the php to process the form with the following as options: post to db post to db and send email to specified address alerting of new form entry send results via email (support of a template to send the form) save to text file in a specified directory provide a "thank you" message or forward to a thankyou page once the form is complete. should create a "project" for the form. ability import a form that was designed and allow for edit via software. should include an installer, that will download all files needed (jquery) and save to a master directory. should include a wizard to start the project creation off if it is not an imported project (existing form) would prefer a visual studio feel also would be interested if this could be built as an add-on for [login to view URL] if you have an existing application similar to this please provide either screenshot or demo.
