PERL script to pipe data through

I need a PERL script that will take FORM data posted to it and pass it through to another URL. This is part of a bigger project that will allow me to build an upload progress indicator. What I need the script to do is take any data that is passed to it and pipe it through to another URL. For example suppose I have an html page like this. Assuming this page works fine and does what it needs to do I want to modify it to post to the needed PERL script, then the PERL script will post to the URL in or example.
