Write an iPhone application

We required to make an app for iPhone. This app is about the live dracaena using our video streaming technology. We already have this live drashan on website and need to make that on iPhone application in following way : 1) User will download the app from store and install. 2) There will be logo and some text to be display. 3) On loading this app user can see the list of channels ( This channels must be in synk with website) . This channel is already working on website with video+audio and only audio and there will be max 3 channels. 4) User can click on the channel and able to view the streaming feed. Streaming feed settings in Jwplayer will be provided by us. User need to upload the full source code and working app before closing the project. We required the documentation of app also.
