Google maps mashup with some customization

I'm looking for a google maps mashup for psychology doctorate programs in the US. It should look and feel somewhat like [login to view URL] except that I would prefer the sidebar to be on the left and checkboxes along the top. In terms of customization, I need separate checkboxes for Clinical/Counseling/School/Combined as well as Phd/PsyD. For instance, users should be able to choose to display all Counseling PhD schools or all Clinical PsyD schools as well as all other possible combinations. I would like these checkboxes to be on top and the list of schools that meet the user's criteria in the left sidebar. I would also like Google Adsense to be integrated into the maps as well as the sidebar. Would highly prefer someone with a proven track record working with Google Maps.
