1 page info sheet created

I need one page form that enables my phone operators to collect info we need for new customers. I have all of the data fields that are needed outlined in a spreadsheet (with dropdowns for possible choices in many of the fields) that would need to be included in this 1-pager. This is what I need from an expert Freelancer: I need the info provided to be arranged in a very nice and neat 1-pager that my phone operators can use easily. The user must be able to “Tab or Shift/Tab” from one field to the next (beginning to end) so that the user can fill out all the fields quickly from start to finish. This can be done only in Excel (and hopefully someone can show me if it is possible to tab to the beginning of the next row from the end of the last one..if not, we'll live with it), or another program that can be converted back into excel (i.e. PDF?) because we need to use the data from this 1-pager to upload into our customer database from excel. I need it to arranged neatly and in the sections that I have it in already. Many of the fields have drop-downs that are populated by the second page of the sheet and I need the one page form to have the same dropdowns (AND BE EDITABLE BY ME SINCE SOME OF THE DROPDOWNS ARE NOT COMPLETE YET – THEY WILL BE COMPLETE ONLY OVER TIME). The 1-pager must be printable also, and flow neatly and in an organized manner. Basically, the reason I need someone is that what I put together (in excel) looks terrible, and it doesn’t flow quickly for someone to fill it out. You kind of have to be an artist with a flair for making something super easy and efficient to use!! This 1-page customer info sheet will be used (by a script) to populate into our contracts we use with our customers, so we will need for the fields to be recognizable to a script/program we are writing. Most importantly, I’d like someone who I can build a relationship with, who’s going to be around for several years that can update this sheet from time to time as necessary. I have tweaks to make all the time, and other projects as well. Once I feel comfortable that you are the right freelancer, I can send you over the Excel sheet I have organized the info in. You can use my sheet as a starting point. Finally, I need someone to get on this ASAP. I need it done quickly because a couple other things are hinging on this for me. Thanks!
