Recreate Form

Hi. Please, recreate (or edit) the attached [login to view URL] file. 1) If you use AppleWorks, you can simply use the attached ZIP file ([login to view URL]) to make the changed. 2) Or, you can recreate the file in Adobe Illustrator. 3) No other program is acceptable. It is either Illustrator or AppleWorks. 4) The attached ZIP file has the original CWK (AppleWorks) file and fonts (Mac). 5) You HAVE to use the same fonts. If you use a Mac, you can use the attached ones. If you use a PC, you'll have to search for the fonts. 6) The attached PDF file has the description of the necessary changes. In summary: 1) Add email field to "Mother or Guardian" by dividing the "Occupation" field in half. 2) Add email field to "Father or Guardian"... 3) Add "Primary Insured" field to "Primary Insurance Carrier" by dividing the "Other Insurance Carrier" field. 4) Update the bottom right of the file: "Patient Identification, Revised 02/17/2010".
