Custom Webform For Wordpress

We need custom webform with specific fields ( will be provided , see image for example) that is wordpress friendly and non theme specific. Form must be wordpress compatible and non theme specific. Must be able to work with 95% of the themes on the market. Form will be placed in the main content area of a post of page. **Note:** Please do not bid on this job unless you are very confident with building code for wordpress. Some of our requirements listed below.... * Form must be able to expand and contract upon different content width of wordpress sites. Final form will have fixed width. But want it built to with the ability to change widths easily. * Form boxes must have nice web2 style rounded corners * Form must be compatible with the top browsers for compatibility. * Form will have Titles and subheadings, form fields, drop down menus, radio buttons, check boxes, text areas and buttons * clickable tooltip icon- pop out when user clicks icon * Form will have outer container that will have bg color. * All elements will have to be quality clean code and structure. * All css must be form specific and not rely on any default css of wp. * NO frames allowed Worker awarded will need to have good communication skills and preferably skype but not required. Worker must also be willing to fix or update any part of the code or design if small changes are needed throughout the build process.
