Dear Client, I have gone through PDF file you have attached .I can do this perfectly. I will develop you a web based application for this purpose. So the user doesnt need to download anything. The user can be taken to the webpage by scanning QR code or entering the URL. Once the user reaches the page, he will be asked to login with username & password (I suggest to use login first instead of the screen 1 you provided. Because anybody who gets the QR code may reach the page and may enter unwanted data. So providing a login will be secure. )I will give you an admin page for you from where you can add/delete/edit users etc. Another advantage of giving username is that, in the screen 1, the nurse doesnt have to type the hospital name, etc every time. The hospital will be auto displayed depends on username. The surgeon name can be selected from the drop down list From the next screen onwards, all functions you mentioned will be implemented in a proper way. The web application I am proposing you will be made with latest coding technologies, so that it will be responsive and compatible to all devices and browsers. Once the data entry is complete, you will get all the required details in the email you provide.
