Simple website login page with SQL

I could do this login page for you, but I have 2 question: 1. What actually you use, ASP.NET WebForms or ASP.NET MVC? 2. Have you already really had this page graphically? I mean is attached picture is "finish variant"?

This seems to be a relatively simple process, easily done in the timeframe given. I have a large amount of experience in ASP development, both in C# as well as in VB.NET. Dear Hirer, Please find my understanding of below requirement. 1. Login implementation, connection to the MS SQL Db and validate whether it is valid. If not valid show the error message to the user. 2. On successful authentication, redirect the main page. 3. Forgot password, implement the forgot password by capturing the email id and check if it is valid. On valid send a mail with reset link, while clicking the user will be directed to the page where the user will key in new password and submit. Then it should go to the login page. Surely I will able to work on this, please provide me the developed pages. I will implement within in a day. Thanks
