407331 Patient Files Database

Using MS Access 2007 create a database visual basic form so that Patients details including name, address, email Phone, Moblie, doctors name, allegies, medication and then date of visit and patient notes which would be held in a hyperlink with each visit being held as a sperate event in a lsit with each date being able to be clicked on to view the patient files from that visit and a few others are all easily added for new patients or viewed for return patients. Have four forms that are present by hyperlink at the bottom of each patients on database being diabetic, surgery and BMX and ACC. When each is clicked on it will need to open a new form which I will supply details of what each of these forms will have on them. They will then be stored in order of date with newest first under there respective headings. Again these froms will need to appear as a hyperlink with date on them if filled out for a patient on the patients page. An area to input and store doctors emails so that patient treaments can be easliy emailed from the database notes into an email. An area to input the cost in the database charged to patient which will then carry through to excel and give a running total of each day when the day is complete. The database will create a note each time the four year anniversary is up for a bmx and will be able to link the name to a letter that will get sent out reminding them of the need for a checkup. The database will also create a mailing list 1 week before date of births so a 5% dicount can be offered for birthdays again through a mailing list. There may be more but this would be a start. Am happy to create what I would see the forms basic set out looking like without any workings in behind. May be scope for further work as we develop the ideas with this later. Information must be able to re retrived from the database for performance purposes. Patients who have not had a repeat for 7 years are deleted on 7 yr anniversay of last visit. Needs to look releatively nice on intial screen but not a major source. Will be used on individual computer. A quick idea as attachted.
