WHMCS Template fix -- 2

We had a vendor who previously help us to setup a custom whmcs theme. But there is some bugs. (A) Javascript error conflicts that prevent whmcs live chat add on from displaying fields properly. Now the fields overlap the text. [login to view URL] The fix that the previous vendor applied to correct the fields in the live chat appears to be a quick fix. When new products are added, the problem occurs again on those new pages. We need this issue resolve across the ENTIRE theme (B) Now when addons are being added on whmcs , it doesnt appear on the cart. After speaking to whmcs support they have confirmed that "Apparently there is an error with your template. Now add-ons doesnt show on the cart. Please see below comment by whmcs The issue appears to lie with your custom " CH-Cart" order form template, as when I switch to the on of the stock order form templates "boxes" - I do see the addon listed.
