Website designer to make corrections on an existing online WuFoo form -- 2

We are looking for a website designer to make a few small tweaks on an existing site. Specifically we are looking for an expert who has experience with WuFoo online forms (WuFoo by Survey Monkey, Inc.) (1) We need to add two (2) page brakes to an existing form. (2) We need to make minor changes to three (3) current fields on the form. (3) We need to add field rules to three (3) existing fields or other information which may be relevant to residential lease applicants. (4) We need to add our own logo to the form removing the WuFoo logo, but keeping the Newfangled theme in place. We are using a generic website developed by Properties Online to market our rental properties.
