We require a WEB form application to record customer product subscriptions. Details of customers are to be recorded and the subscription / product that they have purchased. Customers can purchase the same subscription year after year. The entry of information will be used to survey customers with 5 or 6 questions to be answered by free text and a numeric score. The survey is done via a Call Centre staff. This web application will require at least 4 or 5 security levels for the WEB forms eg. (1) No access - invisible form (2) Read Access (3) Read and Update access but no delete (4) Full Admin access. The information in the database will be used to create Mail Merges and emails so the back end database will need to be accessible / readable / updateable via an ACCESS link. Currently the business structure is kept with multiple EXCEL spreadsheets and TABS within the workbooks, so we expect that a database could be designed from this information relatively easily. I have a long and experienced background in web / database applications and will be doing the TESTING and QA and I will be the contact for the application development. The web application will not be a standalone application but will need to be easily called from a conventional WEB site when customers choose to purchase a subscription. The WEB app forms will need to accept a 'Style sheet' so that this web app will look like it belongs to the parent web site. We have a realistic expectation of the size and complexity of the application. We would need to retain source code and copyright of the developed software.
