I want a form that takes a user's name and signature then saves this information, along with date, time and ip address to a MySQL database. The signature should be base64/svg. There should be an index page where they sign. Here there should be the ability to clear the signature through a button and a submit button to 'post' to database. There should be a separate page where the information gets processed, checked and verified prior to upload. A third page will be able to PULL the information from the database by selecting the users id/name generated from a database query of usernames/ids.
