Build a php form to generate reports for 3 levels of administration

Hello Freelancers, We will need a php form for Sales Reps to input the following: 1. Customer Information (name, address, contact info etc.. with textboxes, and multiple selections) 2. Product List (With name and product code) 3. Product Order Info (Units, quantity, customer who ordered it, price, excahge rate, discount, tax etc.) Then this form should be able to generate a report to notify Sale Coordinator with e-mail. Then after Sales Coordinator autenticates and he should be able to use a tick box for this and drop down a small note on it. After Sales Coordinator authenticates the report then it will notify the General Manager via E-mail. After General Manager authenticates and he should be able to use a tick box for this and drop down a small note on it, the order will be complete and will be send to wharehouse for shipment. This process needs to be stored on the website database and should be able to use filters by sorting by date, product and customer. Also the databse should have a seach function to find date, customer or product information. For all this, we have worked with asp and about to finish the project.
