Form script in PHP+mySQL Edit

Hello. We urgently need a php based form script for clients to contact us and calculate costs (simple calculations). The script should run with AJAX and with a nice web 2.0 GUI. Please use PHP and mySQL for datastorage, for email user the builtin mail (sendmail?) You may use existing code from the web. 1. User enters the script via html/php 2. User is presented with a progress bar with 6 steps in the top of the form. It is highlighted on step 1 3. User sees form slide nr 1 in the middle 4. User sees a footer with Back - Next buttons PM me with your bid, LETS COMPLETE THIS OVER THE WEEKEND! I will present you with the slide contents (forms required..) UPDATE: This script is now completed, we now need minor tweakign to it. It is in javascript with jquery and ajax. The other developer is gone missing, and we now need a good developer to finish it. Things we need changed: 1. Calulation formula (one line of code we believe) 2. Add another field to db/script. Should not take more than one hour :) I will send you the script and db so you need a php webserver to show me your demo on.
