Personal Birth Card with GD Graphics Library

- I need a very simple page with an form. - In that page we will put: customer name (receiver), his e-mail, my name (sender), my e-mail. - Then, the page will save a "birthday card" with the customer name incorpored in the image. The card must be saved in the host. - Then the card will be Sent to customer and sender, like embedded image through php mail. - images must open in outlooks and webmails. - In attachment you will see the card mockup. Where you read "mary jane" will be the customer name. - The customer name must have the same font of the text. - I will send to the developer, a photoshop file with this mockup but I will customize that after the job was done putting the logo and other details.
