JavaScript to PHP port - repost 3

I have the source code for a few different programs that I want somebody to take a look at and take a look at this simple (One-Page) website. The website literally is only a page and it is a program for Bitcoin Escrow System. I need the website looked at and I want the source code looked through thoroughly and ported over to PHP, specifically version PHP/5.4.4-14+deb7u5. When you see the website, you are going to see it is a very basic website and could probably be ported over to PHP in just a couple solid hours of coding. You can use any PHP library that you need, although if you can write the code without a library, that would be even better. Just raw code, but I am looking for someone to give me a quote. I DO NOT want the Bitcoin Address Utility ported over to PHP, I just sent you that source code as a reference because it is the original raw source code that [login to view URL] used and modified for their website. As you can see, this website ([login to view URL]) looks like it was programmed in the 80's or 90's. When this project is finished I want the PHP version to look like it was programmed this year using the best technology and coding techniques. Make it look like a 2014 website!! That website just looks mediocre and has NO flash or style to it at all..... I am not looking for a slow sight with a bunch of bells and whistles either, just keep it simple and make it look 1000x nicer and more professional than the site listed above. Upon completion of this project, I will need the entire source code in it's entirety and any and all PHP libraries that were used in the development of this project.
