One questionnaire in Joomla

I need a Questionaire, which I have on Paper (pdf attached) set into an existing Joomla-System. The questionaire must not be exactly the same, als long as the structure is kept and, if possible, the logo of the company, the questionaire comes from, is present somehow. And the order of the questions should keep the same. It´s four pages all in all, I would prefear it if somebody can set it in FacileForms with also four pages. FacileForms is already installed on the platform. So if somebody is fluent in FacileForms for Joomla this is a quick and easy thing to do. The questionaire should be open for everybody, also for guests. If possible it would be great, if logged in users fill out the questionaire, the userdata as it already exist (Name, Adress,....) should be pre-filled and if a user changes it, it should also be changed in the user-database. The Questionaire is in german langague, but it should be easy to understand, how the structure is, and there is no need to understand all the questions. However, if you want to know, I can translate them. ;-) The results should be sent to an eMail-Adress after the Form is filled and there should not (!) be a statistic vieable, but bouth of this are standarts in FacileForms. The real thing is that I´m just not fluent enough and to lazy to set it all in FacileForms. ;-) Oh ... one more thing. My website is a full active community, so I would prefear if there is a way that you built it on your page so I can import it later, instead of building it up in the live-system. All the components (Joomla, FacileForms, CB,...) are GPL. **A little Addition**: I have Acajoom installed as my Newsletter-System. I´d like to have the option "subscribe Newsletter" prechecked and when a guest fills out the questionaire and keeps this option he should also be copied into the newsletter-subscriber-list. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
