Self Development author needs ebook formatted for online distribution

I need someone to format my ebook for online distribution into both .pdf and .epub formats. 1. I will send you an image of the book cover which needs to be inserted as the first page of the document. 2. The main fonts used are Montserrat and Open sans. Montserrat is featured for the titles and headings, and open sans for the text. 3. The files need to be formatted so that they appear readable on a computer, e-reader and tablet devices (i-pads etc). 4. The contents table needs to be hyperlinked so that readers can click on the chapter link and be taken straight to the chapter. 5. The document needs to be spaced properly so that Chapters begin on a brand new page. I would prefer this for new headings as well, as long as there are not excessive gaps. 6. The 3 sub-sections of the book have not been included in the chapter, please advise if it’s standard to do so 7. The document features exercises which i have written into boxes. If there is anything you can do to make these look better i’d love to see your suggestions. 8. The page numbers and footer need to be kept in the document. I will require revisions to make sure the formatted document is to my satisfaction. Files will be sent through upon selection of a freelancer Budget is $20-$30USD.
