Transcribe PDF text + handwriting on PDF to plain text

This is a very simple project but I want to make it clear for you to understand. I have PDF copies of questionnaires/surveys with 35 questions. The respondents answered the surveys in handwriting. I need 1) the original PDF of the questions transferred to plain text (easy copy and paste), then 2) the hand-written answers transcribed within this plain text, under the corresponding question. Other notes: 1) in the case of multiple choice, list the answers as the text of the selection checked/ticked. 2) Each respondent's initials will be on their copy. You must note the initials next to each answer so as to differentiate between the answers. How much work is involved: There are two sets of surveys (same questions, different context in which they are being answered, thus they must be separated). One has 4 respondents, the other has 3. There are about 35 questions, with the responses being only 10 to 15 words each, and some are multiple choice selections with no handwriting. Some surveys have several questions unanswered. Because of how easy this job is, I would expect very low bid amounts on this. NOTE THAT I NEED THIS COMPLETED WITHIN 6 HOURS OF GETTING YOU THE PDFs.
