To create a customised Google map with a list of pin locations from my list of addresses

I have a list of addresses (about 2500+) which I need them to be added on the Google map as a location pin including three lines of text information I.e. a name, a date and an address. Then this map can be sent to me as a google my map. I need this map to be done accurately within two weeks time. If you are interested, please let me know.
