I'm the team leader and in fact I'm a Doctor, so I'll enforce the scientific approach during this project as we do with our usual health industry ones. The email linked to a web site, would be my suggestion, we work with the Microsoft East Coast Datacenter in Viriginia (Microsoft Azure), the cost for maintaining a Web Site like this + an entry level SQL Database on it, will be USD $ 17/month. If the database gets bigger or have much concurrent users, has to be scaled up, we can work with cheaper providers also. There is something that concerns me though, if you want us to create the questions and the filters, that's extremely easy and cheap, we may charge for every change but maybe in the range of $20-35, for every new questionnaire. If you want to be totally autonomous, of course we can do that, but that's a ton of programming, it means we have to develop another layer in top of the Development Platform, where your click commands translate into HTML code, language filters etc. As we are offering you the complete source code of your solution, I suggest to start with the questions and filters written by us, so you have this fast in 1 or 2 days, and then we can do new milestones to develop that extra layer, in your source code.
