ASP Upload to Host

I have two forms already designed. I need them to be linked with an Access database on our host site. The first form would populate the text data into an Access database already out on the server. This form also has an option for the user to upload a Word document that would be stored in a different directory on the server. Once the first form is populated, the user would be taken to a second form, which would only contain text data and would populate into the same Access database. After submitting the second form, we would just like a confirmation page to appear. This should be a very simple project for those who know what they're doing. It needs to be completed by 3:00 CDT today.
