I need a 360 degree evaluation questionnaire developed on Survey Monkey. Does not need to be too detailed. I need the questions to be focused on evaluating an employee, but him/her: 1. Subordinates, reports 2. Peers 3. Leaders So this will be a 3 part questionnaire, coming together as a single outcome. You will need to provide the questions, and I will edit them as necessary and provide it back to you. Then you need to set up a SM survey that can be published and results collated.
