Build an adaptive application form Website for Desktop, Tablet and smart phones.

I am looking for an adaptive web application developer The application is an online registration form. Built on MS SQL 2012. It needs to be adaptive for Desktop, tablet and smart phones. There are three parts of the applications a. Profile; Users and admin user. b. Job opening table listing [Vacancies] This is going to be entered by an Admin user. c. Upload of multiple JPG, PDF or gif images for the profile. There are few components with the application; 1. New user will create a username and password. a. The account will be verified via email 2. Then with a verified account, the user can now proceed to creating a personal profile a. Personal Data, Address, Identification, Relations, Medical, Criminal, Education, Employment history, Civil Service history. 3. Upload certificate images – pdf, jpg a. Note that the upload of images will be from either Desktop, laptop or smart phone 4. Submission will then create a report of the application form and send it in an HTML5 format embedded to the email.
