Customer Satisfaction Survey - MS Office/Access DB

I suggest completing the project in Excel to keep the user experience very simple. To do so we'd build a comprehensive date collection and analysis tool which would include: 1) A clear and concisely designed form on one sheet, easy for the researcher to complete, with options to add and vary questions. 2) A macro to transfer this information with a single click to a database held in an alternative sheet . 3) An analysis page, allowing you to quickly see a summary dashboard indicating dissatisfied customers, number of participants, averages and other relevant statistics and analyses. All represented numerically and graphically. The same tool would be usable for multiple surveys across multiple years, and can be built to include tracking of the satisfaction of returning customers, perhaps red-flagging customers with negative feedback over more than 1 survey. I have extensive experience (over 5 years) in building excel tools of this nature in the past, including models with significantly more complex requirements such as links to multiple databases, macros to populate word documents with text, graphs and tables, and tools used simultaneously by over 100 people.
