Diagnosis DataGrinder

The objective of this application is to be able to collect data on patients and store it in a database that will, depending on the data entered, diagnosis symptoms automatically. example symptom1+symptom2+Symptom3-symptom4= Diagnosis Thats the main function that needs to be done. It will also need to be able to make office visit notes, by grabbing certian "datafields" and forming the note on a per patient basis. I have the entire project drawn up and ready....i just need it put into an application. Preferably an application like Filemaker. Reason for this is so that it can be edited and changed easily in the future, by me. I already own Filemaker Pro 8 Advanced. **EDIT** When i say Filemaker thats just a suggestion. What i really mean is, from experience, this software will need to be changed frequently, and fast. What ever it takes to get it done is fine with me, but just keep that in mind. I have drawn this project out, to what I think is perfect, but theres always something that I missed or figured wrong. I uploaded a file with all of the diagnosis and descriptions. What i have drawn up is the formula to figure the diagnosis, which will be sent to the winning bidder. To get a better understanding of what i want, search google for ComChart....that program does basically what I want EXCEPT..that program has way to much stuff....I just want something to do my progress notes, and store TONS of data on patients the way I want it stored. Currently I use NDCLytec Medical Software which runs off Progressive SQL, so knowing progressive sql will make this an even easier job because 35% of all of the data is already in a database, it just needs to be formulated into what i need. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
