Combined .NET patient database and basic program

I require the development of a database using MS Access and . NET framework -preferably VB.NET. The program will combine a very simple paint program with a customizable database. The program will have 3 major sections. 1. A menu page containing options: Database customization options Database searching/queries Database new entry User profile Job List Check for updates section online linkup Purchase full version ??" I have a multi-level trial/full copy protection program .NETcompatible DLL to be supplied 2. The working page: This page will have a tool bar with various drawing tools (attached). This screen will have a diagram set as a background screen. The diagram will have designated regions that will be used to collect data for a database. When a user draws a figure over the diagram, the coordinates of the figure are located to a designated region and are logged in the database. Some figures will have additional preset, user customizable values selected via small drop down menus when a figure is drawn ??" these region specific values will be automatically entered into the database. 3. Database: I need suggestions about how best to structure the program and database that will allow for greatest speed and customizability ??" thus I would like someone with significant experience with database design. The main structure of the database will remain relatively constant but I want to be able to add and remove a variety of different fields and sub fields. I want to be able to have set fields that can be created by users that can act as templates and populate the working page as above. The templates should be easy to create e.g. wizard . Programmer should have excellent English language skills, not be using an online text interpretation service and be available via phone, email, fax, MSNMessenger / Skype. They should be proficient in graphics and database areas.
