An interactive survey funnel script

I would like a script that can be used in html pages or uploaded to server or a wp plugin. The script would make a one page survey of one question that must be interactive if the user would fill out yes or no. The yes button directs the user to another link then the no button. As product seller you can direct your customers more targeted pages or links. Layout is essential. Must look and feel good. Would be nice if eveything can be changed. Layout and buttons.
