Create personality test in Wordpress

Hi, Goal: have a personality test with a end score based on four personality types (DISC). The test exists of 10 'questions'. Each question has 4 answers. Each answer needs to be rewarded with a score, the available scores are 1, 2, 3 or 4. The score of 4 is the best fit, 1 is the least fit for the person. It is not allowed to use a number twice. Each answer corresponds with a personality type: Dominance, Influence, Steadiness, Compliance. EG: Question 1 - Optimistic - Self confident - Exact - Well balanced The answers correspond with the four types: - Optimistic = Influence - Self confident = Dominance - Exact = Compliance - Well balanced = Steadiness A possible answer to this question could look like: - Optimistic = 4 - Self confident = 1 - Exact = 2 - Well balanced = 3 The scores of all questions need to be added up from all questions to the right personality type. These result in a total score per personality type. Based on the score the answer to the person would be: "It looks like your preference is [..] en the [..] as second. Generly spoken the [...] can be explained as [..] Requirements: e-mail the score to user possibility when sending score, add e-mail address to a mailchimp list - based on the result on one of four lists use css of my website all questions on one page answer & result text need to be editable by me (no problem if this is needed directly in the php code) use a shortcode in a page (it must be possible for me to add extra information the page.
