Hi there, Can we use coldfusion for build this form and embed it into your wordpress page? Coldfusion is owner of adobe so it's very easy to build a fill-form like your attachement pdf file and after that i save to to your mysql db. Best regards Thanh

Hi, The steps involved as fairly straightforward: 1. Create a new PDF form in Acrobat (Pro); select the existing form as a basis 2. Insert, as appropriate, the required checkboxes and text fields. 3. "Close Form Editing" 4. "Enable Adding Text In Documents", etc as needed. 5. Test in Acrobat Reader. 6. (Likely) Tweak new form again. 7. Repeat 5 & 6 until the form is adequate. 8. Hi there, based on the information provided and by looking at your order form, I can see you are relying on this document to be faxed back to you for manual entry into SharePoint. I have experience doing this type of fillable PDF document for fax processing where I currently work a large investment company in Canada. Here is how I would go about this project as a fillable PDF: Created in Adobe Acrobat X - compatible with all browsers and programs for reading PDFs 1. You would have a big mix of fillable fields for data such as name, date and mobile number and a big range of checkboxes. Some fields will be restricted to numbers and by length. 2. I would consider using big black squares instead of "X"'s for the check boxes in the form, as the squares are easier for your processor to read after being faxed. X's are doable if you like. You can pick which you prefer. 3. Checking certain checkboxes, and clicking on certain fields will erase other checkboxes, to restrict selection. Example, The first three checkboxes. I assume you can only select one. 4. Some of the data from the top of the form such as customer name would automatically feed to the second page. 5. Make it easy to "TAB" through 6. Create 4 color appropriate (but your choice !) buttons to easily click that will email the form, save the form, erase the form or print the form at the top corner of the document. I would finished with a few quality control checks, which is pretty key with these. Thank you Shawn
