Bot - fill form - get result

I need a script (robot) that would do the following: - connect to a website - fill a form that has about 20 fields on 5 pages (4 fields, click next, then 4 more fields etc.). (Some of the fields are drop/down controls and radio buttons) - click "submit" - then get and store the result of the form submission ("accepted" or "rejected") (repeat those tasks as many time as needed) The data for the forms should be read by the script from an Excel or SCV file. The output of the form submission (accepted/rejected) should be stored in a basic DB or in a flat file. Your quote should include installing your script on my server and delivering it to me "ready to use".
