Create Simple Website Form

This should be very simple for an experienced coder. We want to create a small form on one of our clients websites. Here is a link that has the webiste code for the home page and includes? as well as a very detailed mock up of what we want the form to look like: <[login to view URL]> We want this form to be on all pages of the site. Becase all of the pages use includes this should be very simple. Just modify the include so the form shows up above the navigation on all pages. This site also already has a working form. You can use the code for that to create a formmail script for this new form.
