Simple 1 activity App where it contains a form. (similar to screen shot) This form will have two EditText for input and one ImageButton and a regular button to Submit. When you click the Image button, user should be prompted to select an image from a source (like in screen shot). When the user selects its image, a thumbnail of the selected image should be visible. When the user is finished filling out the form he will hit the Submit button. When hitting the submit button, it should make an HTTP POST to a PHP file and upload the file and form contents to the server.
