Attached is a simple, 1 page form that I need developed and added to our web domain. 1. The form can't be navigated to from our main website. So someone would need to know the url to see the form. 2. A login is required to complete the form. The login accounts are to be administered by our internal staff (no self service option for users). Document or train us how to create, edit accounts 3. Once the form is submitted the information should be (A) stored on our web server in a csv file and (B) emailed to help"at"[login to view URL] 4. The STATE field should be pre-populated with "Georgia" 5. The USERNAME field should be populated with the login name of the submitter

Hi, for your project i will create a form page for login and one for the form submition. When a user login into the platform will be redirect to the form where username is pre compiled with login username. After form submit data will be added to a csv file with all the datas of other submits. Only one question. One user can send only one time info or not? User informations (username and password) are stored in a txt file because i think that in your webserver there is not mysql. right? For add or modify user you only have to download from FTP this file, make mods and re-upload it. If a non logged in user try to reach the form page it will be informed that the page is available only after login. Waiting your replay. Thanks and have a nice day. BR Michele
