Create a simple squeeze page / landing page for me in Wordpress

I would need a real simple squeeze page / landing page created for me in wordpress and hosted with my godaddy account. It would be as simple as this one - [login to view URL]$1ToiletryBag/ I prefer wordpress to enable us just add the mail chimp plugin for opt-in form and set up the back end in my mail chimp account. I would need you to do this too including creating a simplistic template (very simple design). This is not a big project and if you are experienced, this should take you less than 2 hours to do the whole thing. Budget is only $15. Please do not bid higher or your bid won't be considered. I can do this myself if I had the time. But have multiple things running in parallel. Looking to pick someone in the next 1 hour.
